Have Your Say! - New Moving On Service
Following an analysis of the current services and feedback received from parents and carers, it’s clear that the current transition process into adult services is not working.
In order to try to address this, a new service called the Moving On Service will be established to work with people aged 18-25 who are eligible for support from Wiltshire Council social care.
As part of the development, Wiltshire Council is undertaking to engage with parent carers to explore and discuss the proposal in partnership with Wiltshire Parent Carer Council and Carer Support Wiltshire.
Three daytime face-to-face sessions and an evening virtual/online session will be held.
There will also be an online consultation page hosted by Wiltshire Council.
